Progress you can see, as a shape
A dedicated view turns the assessment into pictures. Each domain is drawn as a radar shape whose spokes are that domain's own statements, with three overlays: where the practice started, where it has reached, and the agreed target. The shape growing outward towards the target line is the improvement story at a glance, and it is the thing people remember.

For the commissioner: the whole programme, live
The commissioning organisation gets a primary care improvement dashboard that is always current: headline figures across every practice and cohort, a needs-attention list that flags what is slipping, drill-down filters by cohort, area, lead and practice, per-cohort progress radars, and click-through to any single practice. It reads the same live data the advisers are entering, so it is never out of date and there are no files to collect. Monitoring a whole programme from one screen is not realistically achievable in spreadsheets.
